Macally UH-275 Driver. I got this from Macally's site (http://www.macally.com/techsupport/drivers.html). It appears to be the only PC drivers for this card. I assume it contains drivers for more than just 98SE but I haven't run the file yet.
File Contents: Their site calls the card a "PCI to USB2.0 Card Adapter". There is also a part number on the card of:
